What Next?

After completing the packet please visit our practice as soon as possible so that we may begin requesting your medical records. After your records have been collected you will receive a phone call informing you of our schedule for your Initial Evaluation. 

  • We require 60 Minutes of your time for your initial visit.

  • Expect a delay of 3 or more weeks to be evaluated.
